| BankSA Margin Lending Application Form Download, complete and return this form to apply for a BankSA Margin Lending facility. These are the terms and conditions that cover the operation of your margin lending facility along with the Risk Disclosure Statement, Power of Attorney, Background on CHESS and Privacy Statement. |

| Asset Access Application Please complete this form if you wish to use equity available in your BankSA Portfolio Loan to support your margin loan. The Asset Access Terms and Conditions are to be read and understood in conjunction with the BankSA Margin Lending Facility Agreement. |

| Cash Management Account Form Complete this form to authorise the opening of a Cash Management Trust Account (CMA). The CMA will be opened in the name of Value Nominees Pty Limited, a wholly owned subsidiary of Westpac Banking Corporation. |

| Managed Fund Standard Transfer Form Provide Managed Funds as security for your Margin Lending Account. |
|
| Share Transfer Form Use this form to lodge your Issuer Sponsored and CHESS Sponsored shares onto your margin lending facility. |
|
| Off Market Transfer Form Use this form to lodge/release your Issuer Sponsored and CHESS Sponsored shares onto your margin lending facility when there will be a change to the beneficial ownership. |
|
| Issuer Sponsored Shares Change of Name Correction and Indemnification Use this form when the name for your Issuer Sponsored shares you wish to lodge to your BankSA Margin Lending Facility is not an exact match to the name on your BankSA Margin Lending Application. |

| Issuer Sponsored Shares Change of Address Notification Use this form when the address for your Issuer Sponsored shares you wish to lodge to your BankSA Margin Lending Facility is not an exact match to the address on your BankSA Margin Lending Application. |

| Credit Limit Assessment Form Use this form when you would like to apply for a credit limit increase or where BankSA Margin Lending has requested an updated financial assessment from you. |

| Statement of Position You will need to complete this form if you wish to write call options using your margin loan facility.You may also need to complete this form if we require further information from you to set up or maintain your margin loan facility. |

| Managed Fund Redemption Request Complete and return this form if you wish to sell part or all your managed fund investment(s). |

| Drawdown Request Use this form to arrange cash withdrawals from your Margin Lending facility. |
|
| Margin Lending Direct Debit Request Debit your monthly interest charges directly from your bank account. In completing the direct debit form request you are accepting the terms and conditions of the Direct Debit Request Service Agreement. |

| Fixed Loan If you wish to fix your loan and prepay your interest complete and sign this form. |
|
| Tax File Number or Australian Business Number Form Complete this form if you wish to provide your Tax File or Australian Business Number and have not already done so. Please note you do not have to provide these details but if you choose not to, tax may be withheld on any interest earned on your Cash Management Trust Account at the highest marginal rate plus the Medicare levy. |

| Update my details Please use this form to ensure we have the correct and most up-to-date contact details for you. |

| Authorised Signatory Form Complete this form to give a person other than your Adviser the authority to act on your existing margin lending facility in all matters as if they were you (except for the receipt of margin calls), or, if you wish to change/revoke this authority. |

| Nominated Adviser Form Complete this form to add/change or remove an adviser on your current margin loan facility. |
|
| Account Closure Form Complete this form if you wish to terminate your margin loan account with us. |
